Payment is expected at the time of service. To better serve our patients
and minimize costs, please note that a missed appointment will result
in the following charge:
Office Visits - $50
Procedures - $100
To avoid these charges, please reschedule two working days (48 hours)
before your appointment.
We gladly accept cash, credit cards, or checks.
Appointments:
Scheduling is a critical issue in every physician's office. PMC is no
exception.
Our physicians' schedules reflect the amount of time necessary to assist
you with your medical issues. While our staff makes every effort to stay
on established schedules, there may be occasions when you have to wait
due to an emergency. We will make every effort to keep delays to a minimum.
Cancellation Policy. Please call (239) 593-9594 to schedule an appointment.
For New Patients:
Please arrive at least 15 minutes before your appointment, allowing ample
time parking.
You will need this time to complete and sign any forms, and time for our
staff to make the necessary copies of your insurance information. You
may wish to call on the day of your appointment to determine if your physician
is delayed. Please make sure you have your current insurance card and
a photo I.D. (driver's license) when you arrive for your appointment.
Many insurance plans require a referral from a primary care physician.
Pain Management Center of Naples, P.A. accepts most insurance, however
we ask you to confirm at time of scheduling your appointement.
For Established Patients:
Each time you visit one of our offices, please make sure we have the most
current insurance information and demographic information (address, phone
numbers, etc.) Please arrive at least 15 minutes before your appointment,
allowing ample time. If you are more than 30 minutes late for your appointment,
you may have to reschedule.You may call our office to determine if your
physician is delayed.

Asked Questions:
1. How can I see a doctor?
You can visit us by asking your doctor for a referral. We work in consultation
with your physician to formulate a successful plan. If you don’t
have a doctor, you can self-refer.
2. What should I bring to my first visit?
If possible, please bring your X-rays, MRIs reports, any relevant previous
studies or procedures, a list of current and past medications, and your
last doctor’s note. We can obtain this information, but doing so
may result in delays. A questionnaire will need to be completed prior
to our interview, so arrive 15 minutes early to allow enough time to complete
it.
3. What do I need to do if I am getting a procedure done?
Do not eat or drink 8 hours prior to the procedure. Stop using aspirin
8 days prior. Call us as soon as possible if you are taking any other
anticoagulants (blood thinners). Have someone available to drive you home
and monitor you for at least 6 hours, afterwards.
4. What insurance do you take?
We take or work with almost all insurance carriers.
